Instagram checks the account
Each time a story is requested, Instagram checks which account is asking. For a private account, it sends the story only when that account is an approved follower.
A website has no approved account
A viewer asks for stories the way a visitor with no account does. For a private account, the answer to that request is the header and a lock notice.
Close Friends is a second, smaller list
An owner picks the accounts on their Close Friends list by hand. A Close Friends story goes to that list and nobody else, even on a public account.
Every approved view carries a name
An approved follower watches from an account, so their name goes on the owner's viewer list for 48 hours. There is no unnamed way in.
The header stays public. The stories do not.
Setting an account to private hides its content, not the account. Anyone can still see its profile picture, name, username, bio and the three counts, and a viewer shows that header with no login.
What a visitor does not get is any story, highlight, post or follower list. The lock is the same for a stranger, a viewer site and an account whose follow request is still waiting.
